- W-9 Form (fillable format) -We pay patronage in the form of a dividend check each year based on our fiscal year financial reports and your purchases. The W9 form is a patron consent form that requires your Social Security number in order to report this information to the IRS when a patronage distribution occurs. We are unable to pay dividends if this form has not been filled out and sent to us for our records. Attach the completed form to an email sent to our office, credit@newhorizonsco-op.com.
